Make an Appointment with the Center to Submit Donations
Call
(415) 479-5710 to make your appointment. Appointments available
Monday - Friday
4.18.08-5.2.08. The Team Leader is responsible for ensuring
proper individual
registration/ donation submission, all team registrations and
donations should
be turned in simultaneously.
 4.
Gather/Collate Completed Forms and Donations
Use
the Transmittal
Form to track donations collected by your Team.
Make a copies for
you records.
5.
Turn In Donations to the Center, and Register your Team members
for the Race
Bring
checks, donation/registration forms, and transmittal forms with
you to your appointment.
Corporate matching gift documentation forms MUST be turned with
your
donations, in order to be counted towards totals.
MATCHING
GIFTS: Many employers sponsor matching gift programs and will
match
any charitable contributions made by their employees.

Check with your company's human resources department to find out
about its
matching-gift policy.
If
your company is eligible, request a matching gift form from your
employer, and send
it completed and signed with your gift. We will do the rest. The
impact of your gift
to this organization may be doubled or possibly tripled! Some
companies aslo match
gifts made by retirees and/or spouses.
6.
Plan for Race Day
Plan
a time and place to meet your team, distribute t-shirts and bibs.
Some groups plan
a post race celebration, like a picnic or a potluck.

Race Follow Up
Send
letters and say thank you to those who participated. Donations
submitted by Race
Day will be returned to returned to your chosen organization/school
by June 30,
